This field-based role coordinates care across the clinical network, evaluates members, develops individualized care plans, advocates for patients, and collaborates with nurses, physicians and care coordinators to improve health and psychosocial outcomes.

Qualifications

• Active and unrestricted RN license in good standing.
• RN with 4+ years of experience.
• Proficient with Microsoft Word and basic computer skills.
• Ability to travel locally up to 75% for patient home visits.
• Strong communication and HIPAA confidentiality.
• Ability to adapt to changing work environments and meet goals.

Responsibilities

• Screens high-risk members for care management needs.
• Perform evaluations and onsite visits in multiple settings.
• Develop and document individualized care plans.
• Coordinate with providers and interdisciplinary teams.
• Ensure documentation complies with CMS regulations.
• Educate and coach patients and families.
• Maintain EMR records in a timely and accurate manner.
• Advocate for member needs with the care team.
• Refer members to community resources as needed.
• Collaborate with MDs and other clinicians on complex cases.
• Manage caseload efficiently and effectively.
