Job summary

UnitedHealth Group is seeking an OptumCare Medical Behavioral Integration Nurse Case Manager (MBI NCM) based in Layton, UT. This field-based clinical liaison will assess plans, implement, coordinate, monitor and evaluate care to meet members' health needs, coordinating with providers and the CMS-regulated care team.

The role requires an active RN license and 4+ years of experience, with strong communication skills and the ability to travel up to 75% for home visits.

Qualifications

• Active and unrestricted RN license per state requirements.
• RN with 4+ years of experience.
• Proficient with Microsoft Word and basic computer skills.
• Proven ability to maintain direct and open communication with all levels of the organization.
• HIPAA-compliant handling of sensitive information.
• Ability to travel locally up to 75% for patient home visits.
• Driver's License and reliable transportation.

Responsibilities

• Screens and identifies high-risk members who will benefit from care management services.
• Performs member evaluations and onsite visits in multiple settings (telephonic, home visits, clinics, hospital, SNFs).
• Develops individualized care plans and documents CMS-compliant plans of care.
• Utilizes clinical skills to coordinate health, behavioral health, and psychosocial needs.
• Documents progress in EMR/proprietary database in a timely and accurate manner.
• Advocates for members and families with the health care team.
