Job summary

UnitedHealth Group seeks an OptumCare MB I NCM to serve as a direct clinical liaison between OptumCare/Care Delivery customers and the clinical network. This role coordinates plans, monitors outcomes and uses education and communication to promote quality, cost-effective care.

If you are located in Washington, you will have the flexibility to work in the field/remotely as you take on challenging cases and collaborate with nurses, physicians and care coordinators to deliver patient-centered care

Qualifications

• Active and unrestricted nursing license.
• RN with 2+ years of experience.
• Proficient with Microsoft Word.
• Strong written and verbal communication.
• Capable of HIPAA-compliant handling of sensitive information.
• Willingness to travel locally up to 75%.

Responsibilities

• Screen high-risk members and coordinate care management.
• Conduct member evaluations and on-site visits across settings.
• Develop individualized care plans and CMS-compliant notes.
• Provide coaching and education to promote outcomes.
• Document status, diagnoses, meds, goals in EHR.
• Collaborate with interdisciplinary teams and timely referrals.
• Support members and families in decision making.
