Job summary

OptumCare in Layton, UT is seeking an experienced MBI Nurse Case Manager to act as a clinical liaison within the Care Delivery network. This field-based role involves evaluating, coordinating, and monitoring care plans for high-risk members, with extensive collaboration across nurses, physicians, and case coordinators.

You will manage patient health and psychosocial needs, document in EMR, and travel to patients' homes up to 75% of the time.

Qualifications

• Active and unrestricted RN license.
• 4+ years of nursing experience (RN).
• Proficient computer skills and Word proficiency.
• Strong communication with all levels of the organization.
• Ability to travel locally up to 75% of the time for home visits.

Responsibilities

• Screens and identifies high-risk members for care management.
• Performs member evaluations and onsite visits in multiple settings.
• Develops individualized care plans compliant with CMS regulations.
• Documents findings in EMR/proprietary database accurately and timely.
• Advocates for members and collaborates with the care team.
• Participates in interdisciplinary team meetings for complex cases.
