OptumCare in Layton, UT is seeking an experienced Behavioral Integration Nurse Case Manager (MBI NCM) to serve as a clinical liaison within the Care Delivery network. This field-based role conducts evaluations, develops care plans, and coordinates home visits to promote quality, cost-effective outcomes.

The ideal candidate holds RN licensure with 4+ years of experience, strong HIPAA standards, and flexibility to travel up to 75% for patient visits.

Qualifications

• Active and unrestricted licenses.
• RN with 4+ years of experience.
• Proficient computer skills and good working knowledge of Microsoft Word.
• Proven ability to maintain direct and open communication with all levels of the organization.
• Proven ability to handle sensitive issues with members and providers in a confidential manner according to HIPAA guidelines.
• Demonstrated initiative in achieving individual, team, and organizational goals and objectives.
• Ability to travel locally up to 75% of the time for patient home visits.
• Driver's License and access to reliable transportation.

Responsibilities

• Screens and identifies high-risk members for care management and monitors utilization.
• Performs member evaluations and onsite visits in various settings including home visits and clinics.
• Develops individualized care plans compliant with CMS regulations.
• Uses clinical skills to meet health, behavioral health and psychosocial needs and provide education.
• Documents status, diagnosis, medications and progress in EMR accurately.
• Advocates for members and collaborates with the care team to coordinate resources.
• Participates in interdisciplinary team meetings and coordinates with providers.
