Qualifications

• Able to travel locally up to 75% for patient home visits.

Responsibilities

• Screen high-risk members and monitor utilization.
• Conduct member evaluations and home/clinic visits.
• Develop individualized care plans compliant with CMS.
• Document findings in EMR/proprietary database.
• Collaborate with interdisciplinary teams for care coordination.
• Advocate for patient and family needs within the care team.
• Coordinate MD consultations for complex cases.
• Apply nursing/counseling theory to improve patient outcomes.
