Qualifications

Responsibilities

• Coordinate all aspects of client care and service plans to assist clients with housing stability and independent living skills.
• Conduct home visits, meet clients in community locations, and document interactions.
• Develop and manage individualized service plans (ISPs) and ensure quality documentation and reporting.
• Maintain accurate client records and support achievement of housing and income goals.
• Contribute to outcomes related to housing stability, income, benefits, and client engagement.
