Job summary

Interfaith Works is seeking a Case Manager to provide housing-focused case management for individuals experiencing homelessness. You will conduct assessments, develop service and housing plans, coordinate with community resources, and maintain HMIS documentation.

Responsibilities include caseload management of ~25 clients, weekly meetings, crisis intervention, and coordination with Continuum of Care. Strong boundaries, judgment, and trauma-informed approach are essential for success.

Qualifications

• Bachelor's degree in social work, psychology, counseling, human services, or related field preferred.
• Experience in case management, housing navigation, or homeless services preferred.
• Fluency in Spanish or other language is a plus.

Responsibilities

• Maintain a caseload of approximately 25 participants.
• Meet with participants weekly and update individualized service plans.
• Coordinate housing applications, services, and community resources.
• Document all contacts and services in HMIS per requirements.
• Participate in interdisciplinary meetings and supervision.
