Zusammenfassung

OneWell Health Care is seeking an IDD Team Lead to oversee daily operations of IDD services delivered in home and community settings. You will supervise DSPs, ensure ISP-aligned care, and uphold regulatory compliance while promoting a culture of quality and person-centered support.

Frequent collaboration with Regional Managers, Families, and community partners is essential to ensure excellent participant care and program growth in Pennsylvania.

Qualifikationen

• Minimum two (2) years of experience supporting individuals with Intellectual and Developmental Disabilities (IDD).
• Experience working within Medicaid Home and Community-Based Services (HCBS).
• Knowledge of Pennsylvania ODP regulations and person-centered practices.

Aufgaben

• Lead, supervise, coach, and develop DSPs, Program Specialists, and staff.
• Oversee daily program operations to ensure compliant, high-quality, person-centered services.
• Manage staffing functions including recruitment, onboarding, scheduling, training, and performance management.
• Monitor participant health, safety, satisfaction, documentation, and service outcomes; ensure timely incident reporting.
• Build collaborative relationships with participants, families, Supports Coordinators, and community partners.
• Support program growth, quality improvement, and audits; represent the organization in the community.
• Coordinate admissions, transitions, service delivery, and communication among staff and stakeholders.
• Prepare operational reports, conduct field visits, and participate in meetings.
