Job summary

OneWell Health Care in Pennsylvania is seeking a dedicated Caregiver/Direct Support Professional (DSP) to provide hands-on, person-centered assistance to individuals with IDD in home and community settings. This field-based role emphasizes daily living support, health and safety monitoring, and promoting independence and community inclusion.

The DSP collaborates with an interdisciplinary team to implement ISP/IPP care plans, maintain required documentation, and participate in trainings.

Qualifications

• High school diploma or equivalent.
• Experience in caregiving or direct support preferred.
• English proficiency required; bilingual a plus.
• Ability to follow care plans and documentation guidelines.

Responsibilities

• Provide hands-on, person-centered support in home/community settings.
• Assist with daily living activities, meal prep, mobility, and meds as authorized.
• Implement behavior support strategies and monitor health and safety.
• Maintain accurate documentation and participate in trainings/meetings.
