Job summary

Betty Hardwick Center is seeking a Service Coordinator to provide comprehensive supports for individuals in HHSC Waiver and General Revenue programs in a multi-county region, including Taylor County. The role emphasizes outcome planning, direct consumer contact, and collaboration with families, advocates, and providers.

Responsibilities include problem solving, documentation, and advocacy to secure needed services, with a focus on enabling consumer rights and well-being.

Qualifications

• Bachelor's degree or higher from an accredited college or university.
• Associate degree in social, behavioral, human service, or health-related field.
• High school diploma with two years of paid or unpaid experience with individuals with intellectual or developmental disabilities.
• Experience with Intellectual and Developmental Disabilities preferred.

Responsibilities

• Provide service coordination for adults and children in HHSC Waiver and General Revenue programs.
• Conduct outcome planning and maintain frequent in-person contact with consumers, families, and service providers.
• Assess needs, negotiate with providers, and advocate to meet desired outcomes.
• Maintain documentation to meet contract requirements and ensure consumer rights are respected.
• Coordinate with community resources to address medical, social, and educational needs.
