Job summary

Betty Hardwick Center is seeking a Service Coordinator to provide coordination of services for individuals in HHSC Waiver and General Revenue programs. The role involves outcome planning and frequent in-person contact with consumers, families, and providers across five counties.

Responsibilities include identifying needs, developing plans with families, monitoring service delivery, and maintaining compliant documentation. Flexible hours and a full-time schedule are available.

Qualifications

• Bachelor's or advanced degree from an accredited college or university; or
• Associate degree in a social, behavioral, human service, or health-related field; or
• High school diploma with two years paid/unpaid experience with individuals with IDD.
• Experience working with persons with Intellectual and Developmental Disabilities preferred.
• Texas driver’s license and insurability; driving is an essential function.
• Proficient in computer applications such as Word, Excel, Outlook.

Responsibilities

• Provide service coordination for adults and children across the Center’s five-county area.
• Perform outcome planning and frequent in-person contact with consumers, families, and providers.
• Identify needs, plan with consumer/family, monitor service delivery, and document thoroughly.
• Coordinate with community resources and advocate with service providers as needed.
