Job summary

Marion County Health & Human Services is seeking an organized, motivated Services Coordinator to join the I/DD team. You will provide case management and coordinate disability-related services across home and community settings.

The role involves facilitating intake, eligibility, and planning, while collaborating with families, service providers, and community partners. A driver’s license and field-work may be required; full-time schedule applies.

Qualifications

• Five years of related education and/or paraprofessional experience in developmental disabilities.
• Knowledge of the public service system for developmental disability services in Oregon.
• Any satisfactory combination of work, education, training, or experience as determined by Marion County.

Responsibilities

• Facilitate intake and eligibility determination.
• Plan and carry out case management and functional needs assessments.
• Coordinate development of Individual Support Plans for K plan funded services in home or community settings.
• Attend IEP meetings and coordinate with schools for transition planning.
• Maintain accurate client records and document services per Marion County policy.
• Monitor services and report Serious Events per ODDS guidelines.
