Job summary

Central Counties Services seeks a Service Coordinator to develop Person Directed Plans and coordinate services under IDD waivers. You will evaluate consumer needs and facilitate planning meetings with providers and families to ensure goals are met.

On-site work is required for the first six months with potential telework later; you will maintain thorough EHR documentation, adhere to timelines, and travel among counties as needed to support consumers.

Qualifications

• Bachelor’s degree in Psychology, Social Work or related field; or an Associates with relevant experience.
• High School diploma with two years of related experience with IDD.
• Ability to travel across five counties and drive as required.
• Trauma-informed care awareness and cultural sensitivity.

Responsibilities

• Develop and evaluate Person Directed Plans (PDP) based on consumer needs.
• Facilitate planning team meetings and coordinate service planning.
• Monitor plan implementation and document progress in the EHR weekly.
• Communicate with providers and families; maintain professional relationships.
