Job summary

Bluebonnet Trails Community Services seeks a Service Coordinator to support individuals with intellectual and developmental disabilities through person-centered planning and community inclusion.

You will partner with families, providers, and community organizations to assess needs, develop plans, and connect people to resources, advocating for access to high-quality services and guiding families through complex systems.

Qualifications

• Associate degree or higher in social, behavioral, or human service field (psychology, social work, nursing, etc.).
• High school diploma with two years paid or unpaid experience working with individuals with disabilities.
• Must meet driving requirements and maintain approved driving status.

Responsibilities

• Collaborate with families, service providers, schools, healthcare professionals, and community partners to ensure coordinated service delivery.
• Conduct comprehensive assessments to identify strengths, preferences, needs, and goals.
• Develop and implement person-centered service plans supporting meaningful life outcomes.
• Coordinate, refer, and link individuals and families to community resources and support services.
• Monitor services for quality and compliance with Medicaid, HHSC, MCO, and agency requirements.
