Job summary

The Harris Center for Mental Health and IDD is seeking a Service Coordinator to help individuals with intellectual disabilities access services and supports through person-directed planning. This role involves coordination with consumers, legally authorized representatives, and providers to monitor progress and ensure appropriate care.

Qualifications

• Bachelor’s degree in a social, behavioral, or human service field.
• Minimum of one year experience in the human services field.
• Experience with persons with intellectual disabilities preferred.
• Must hold a valid Texas Driver's License and be insurable.

Responsibilities

• Complete assessments as specified by department policy.
• Monitor client at least quarterly.
• Update consumer files as needed.
• Complete all documentation within established timelines.
• Comply with safety, health, and training requirements.
