Job summary

The Center for Human Services is seeking a Service Coordinator to manage a diverse caseload and coordinate community-based services. The role emphasizes advocacy, timely documentation, and collaboration with interdisciplinary teams to support individuals with developmental disabilities and their families.

The position requires a Bachelor's degree or RN license, reliable transportation, and access to a vehicle for home visits. A hybrid work arrangement and stable internet access are expected.

Qualifications

• Bachelor’s degree from an accredited college or university or Registered Nurse license.
• Must have reliable transportation and an acceptable driving record.
• Must use your own vehicle for transportation and be able to drive for work-related assignments to individuals’ homes.
• Missouri driver’s license preferred but not required; must have valid driver’s license.
• Proof of personal, valid vehicle insurance required.
• Access to reliable internet for telecommuting employees.

Responsibilities

• Manage a caseload of varying ages and complexities to improve quality of support and services.
• Develop community relationships and link resources to meet individual needs.
• Coordinate federal, state, county, and community programs to obtain necessary supports.
• Monitor and assess service quality, timeliness, and effectiveness with regular reviews.
• Document activities in case notes within three business days and maintain required billable hours.
