Job summary

Achievement Health Group LLC is seeking a Service Coordinator to support quality initiatives and enhance communications for individuals served. You will work under the Manager of Operations to ensure adherence to IDD standards and improve personal outcomes.

The role includes onsite visits, supervising direct care staff, tracking goals, and coordinating scheduling and documentation to meet DHS/IDD requirements. Travel is expected as needed.

Qualifications

• High School Diploma or GED required.
• 3 years of experience with IDD population preferred.
• Strong written and verbal communication and interpersonal skills required.

Responsibilities

• Make onsite home visits to individuals served.
• Interview and hire direct care staff.
• Supervise direct care staff.
• Develop/monitor/track individuals' personal outcome measures and goals.
• Document unusual occurrences and report to operations team.
• Review medical records and ISP data for compliance.
• Assist in staff scheduling and training onsite.
• Transfer data to complete monthly reports.
• Coordinate with team to ensure services follow IDD standards and best practices.
• Oversee programming to ensure person-centered services and dignity of choice.
