Job summary

Achievement Health Group LLC seeks a Service Coordinator to support operations by developing quality improvements and coordinating care for individuals under IDD programs. The role reports to the Manager of Operations and requires travel as needed.

You will supervise direct care staff, conduct monthly reviews, and ensure compliance with DHS, IDD, CQL, and PASSE standards while maintaining person-centered service delivery and strong cross-team communication.

Qualifications

• Demonstrated leadership and the ability to guide teams effectively.
• Excellent oral and written communications skills with interpersonal strength.
• Strong organizational and time-management capabilities.
• Ability to initiate and maintain successful collaboration with staff.
• 3 years' experience with IDD population.
• Knowledge of healthcare laws, regulations, and AHG policies.
• Ability to apply sound, mature decisions.
• Computer experience: Word, Excel, and other software.
• Familiarity with AHG system and IDD standards.

Responsibilities

• Ensure AHG, LLC policies, procedures, best practices, and standards for IDD are followed.
• Make onsite home visits to individuals served.
• Interview and hire direct care staff.
• Supervise direct care staff.
• Review and coordinate individuals' finances.
• Develop, monitor, and track personal outcome measures and goals.
• Document unusual occurrences and report to operations team.
• Review hard copy and electronic files for IDD standards compliance.
• Provide on-site training and staff development as needed.
• Assist with staff scheduling development and coordination.
• Transfer data for monthly reports.
• Coordinate with team to ensure services comply with IDD standards.
• Monitor medication administration and audit MARs.
• Audit medical records and ISP data for compliance.
• Assist with admission records for new individuals.
• Ensure compliance with DHS, IDD, CQL, and PASSE requirements.
• Provide oversight of programming.
• Deliver person-centered services and dignity of choice.
• Complete Provider Reviews during monthly home visits.
• Conduct area team meetings as needed.
• Review site documentation for compliance.
• Perform other duties as assigned.
