Job summary

Independent Living Systems (ILS) is seeking a Supervisor, Care Coordinators (IDD) in Orlando to lead a team of care coordinators serving members with intellectual and developmental disabilities. You will oversee quality, compliance, coaching, and performance, and collaborate with care managers, providers, and interdisciplinary partners to ensure timely assessments and person-centered plan delivery.

The role requires a bachelor's degree, 5 years in care management (3 in IDD), and knowledge of IDD

Qualifications

• Bachelor's degree in human services, social science, or health field.
• Five years in care management; at least 3 years with IDD.
• Strong knowledge of IDD services, resources, and regulatory requirements.
• Proficient in documentation, reporting, and EHR/case mgmt software.
• Proficient in Microsoft Office apps.

Responsibilities

• Lead and supervise a team of care coordinators to ensure high-quality member-centered service delivery.
• Monitor care coordination activities, staff productivity, and caseload performance for timely assessments, care plans, documentation, and follow-up.
• Ensure compliance with organizational policies, contractual requirements, and state/federal regulations through oversight and corrective action.
• Provide coaching, performance feedback, training, and professional development.
• Review dashboards, reports, and data to identify trends and drive process improvements.
• Collaborate with providers, families, and interdisciplinary teams to support person-centered care plans and resolve issues.
• Promote member satisfaction and continuity of care through responsive service delivery.
