Job summary

Lakes-Regional-Community-Center is seeking a Service Coordinator in Texas. The role focuses on assessing service coordination needs, planning and monitoring services, and advocating for consumers while maintaining documentation and communication with families and providers.

Qualifications include a bachelor’s degree or higher, or an associate degree with relevant IDD experience; a high school diploma with two years of IDD experience is also acceptable. Bilingual in Spanish and English is a plus.

• High School diploma with two years paid/unpaid experience with individuals with intellectual or developmental disabilities.
• Bilingual in Spanish and English is a plus.

Responsibilities

• Assessment and verification of service coordination needs.
• Service planning, referral, and monitoring of service provision.
• Advocacy for consumers.
• After-hours crisis services.
• Documentation of progress.
• Frequent contact with consumers, family members, and service providers.
• Flexible schedule including after-hours, holidays, and weekends.
