Job summary

Bluebonnet Trails Community MHMR Center is seeking an IDD Intake Coordinator for the eight counties in Texas. This role involves screening, enrolling, and coordinating services for individuals with intellectual and developmental disabilities.

The ideal candidate should have a Bachelor’s degree in a related field and some experience in case management. The position offers an hourly salary range of $21.46 – $27.71, and additional bonuses for tenure, bilingual skills, and certifications.

Qualifications

• Bachelor’s degree or higher in social, behavioral, or human service fields such as psychology, social work, nursing.
• Valid Texas driver’s license with approved driver status.
• At least one year of case management or service coordination experience.

Responsibilities

• Screen and enroll eligible children and adults with Autism and developmental disabilities.
• Assess needs and develop service plans using Person‑Centered Thinking model.
• Facilitate service linkages and provide crisis intervention.
