Job summary

The ideal candidate will have a relevant bachelor’s degree and experience in case management. Flexibility in hours and travel within specified counties is required.

Qualifications

• Bachelor’s degree or higher in a relevant field.
• Valid Texas driver’s license required.
• Experience in case management or service coordination preferred.

Responsibilities

• Coordinate IDD programs across eight counties.
• Manage enrollment into Medicaid/GR programs.
• Develop service plans based on Person-Centered Thinking.
