Job description

Position Overview

The position serves as an IDD Intake Coordinator for all eight counties (Bastrop, Burnet, Lee, Fayette, Gonzales, Caldwell, Guadalupe, and Williamson). The coordinator is the point of entry for publicly funded intellectual and developmental disability (IDD) programs, enrolling eligible individuals into Medicaid/GR programs: ICF/IID, Community First Choice (CFC), GR Community Services, and Autism Services.

Key Responsibilities
  • Screen and enroll eligible children and adults with Autism, developmental, or intellectual disabilities.
  • Assess needs and develop service plans using the Person‑Centered Thinking model.
  • Facilitate service linkages, monitor effectiveness, and provide crisis intervention and advocacy services.
  • Complete eligibility screenings for ICF/MR, CFC, Autism, and General Revenue streams.
  • Refer individuals to generic resources and to specific services such as ICF/MR, CFC, and Autism programs.
  • Participate in interagency planning, CRCG meetings, and employment‑related projects (e.g., Employment First).
  • Maintain professional, assertive intervention, telehealth or in‑person contact with individuals, families, and providers with minimal supervision.
  • Adapt hours and schedule to meet family needs.
Qualifications
Minimum Qualifications
  • Bachelor’s degree or higher in social, behavioral, or human service fields such as psychology, social work, nursing, rehabilitation, counseling, sociology, human development, gerontology, educational psychology, education, or criminal justice.
  • Valid Texas driver’s license with approved driver status throughout employment.
Preferred Qualifications
  • At least one year of case management or service coordination experience with individuals diagnosed with Intellectual or Developmental Disability.
  • Knowledge of appropriate community resources and experience with Medicaid Waiver programs.
  • Familiarity with the SmartCare program.
  • Bilingual proficiency.
Location & Hours

Remote and in‑person within the eight counties listed above. Some travel may be required.

Compensation

Hourly Salary Range: $21.46 – $27.71, with potential bonuses:

  • Up to $4.00/hour – Tenure with BTCS
  • .75/hour – Bilingual
  • $1.00/hour – LCSW‑A, LPC‑A
  • .50/hour – LCDC Licensure
