Responsibilities include accurate documentation, collaboration with interdisciplinary teams, and adherence to state and federal regulations.

Qualifications

• Experience supporting individuals with IDD/Autism preferred.
• Ability to follow individualized service plans and safety protocols.
• Excellent documentation and communication skills.

Responsibilities

• Provide person-centered support for ADLs and daily living.
• Implement ISPs and BSPs, promoting community integration.
• Monitor health and safety, administer meds if certified, complete timely documentation.
• Collaborate with interdisciplinary teams and attend required trainings.
