Job summary

Klamath County seeks a Services Coordinator to deliver targeted case management for individuals with intellectual and developmental disabilities. You will develop and monitor Individual Support Plans, connect clients to medical, employment, and community services, and ensure Medicaid requirements are met.

Strong collaboration with families, providers, and state agencies is essential to support self-determination and quality of life.

Qualifications

• Bachelor's degree in behavioral or social science, or closely related field.
• Bachelor’s degree in any field plus one year of human services experience.
• Associate’s degree plus two years of human services experience.
• Oregon Driver's License.

Responsibilities

• Assess and reassess individuals to determine service needs and develop ISP/Annual Plans.
• Assist families in developing and implementing a support system.
• Coordinate with state agencies and community partners to access services.
• Deliver person-centered services and monitor Medicaid Plan of Care.
• Maintain confidentiality and professional boundaries.

Education

Bachelor's degree in behavioral science, social science, or a closely related field

Bachelor’s degree in any field + 1 year of human services related experience

Associate’s degree in a behavioral science, social science, or a closely related field and two years of human services related experience
