Job summary

Hearts With A Mission is recruiting a Health-Related Social Needs Case Manager for a full-time, on-site role in Klamath County. You will screen, navigate, and process requests for services, entering data accurately to support timely access to resources.

Collaborate with care teams, maintain program knowledge, and build relationships with community partners to enhance client outcomes and promote empowerment.

Qualifications

• Bachelor’s Degree in Education, Social Work, Psychology, or a related field.
• Minimum of two years’ experience working with at-risk clients.
• Applicants must meet Oregon driver’s license requirements and have reliable transportation.

Responsibilities

• Screening and intake assessments to determine eligibility for HRSN services.
• Coordinate referrals and service navigation between clients, care teams, and partner organizations.
• Enter and maintain data in databases, ensuring privacy and program compliance.
• Verify eligibility and authorization documentation for timely service delivery.
• Collaborate with interdisciplinary care teams to align with service plans.
• Monitor service requests and follow up to ensure resolution and resource connection.
• Maintain knowledge of program guidelines and community resources.
