Job summary

Kern Regional Center is seeking a dedicated Service Coordinator/Case Manager to coordinate community services for individuals and families in Kern, Inyo and Mono Counties. You will develop and implement IPP/PCP plans, prepare progress reports, and conduct annual reviews for clients in traditional and Self-Determination programs.

The role requires effective advocacy, information and referral expertise, and the ability to monitor vendor services while adhering to state and federal guidelines.

Qualifications

• BA/BS degree in human services or related field and two years of relevant experience.
• Bilingual Spanish/English is highly desirable.
• Detail-oriented and highly organized.
• Strong computer skills: word processing, spreadsheets, presentations.
• Strong written and verbal communication skills; customer service oriented.

Responsibilities

• Provide overall service coordination and advocacy for consumers and families.
• Arrange services and develop IPP/PCP plans and progress reports.
• Prepare annual reviews and monitor program effectiveness; follow-ups as needed.
• Identify community resources and provide information and referrals.
• Initiate vendored services and monitor service delivery; stay compliant with regulations.
