Job summary

Umatilla County seeks a Services Coordinator to provide case management and coordinate services for individuals with intellectual/developmental disabilities under the CDDP program. Duties include interviewing clients, coordinating with teams, and monitoring needs within timelines.

Candidates should have a bachelor’s or associate degree with relevant experience, be able to travel within Oregon, pass background checks, and hold a valid driver’s license.

Qualifications

• Bachelor's degree in behavioral or social science or related field.
• Bachelor's degree in any field plus one year of human services experience.
• Associate degree in behavioral or social science with two years of human services experience.
• Three years of experience serving individuals/families with significant support needs.

Responsibilities

• Provide case management services to eligible individuals.
• Facilitate meetings with individuals, their teams, professionals, and contractors.
• Identify physical, social, medical, employment, and other needs based on gathered information.
• Complete and update annual plans, risk assessments, functional needs assessments, and other required documentation within timelines.
• Authorize and monitor services and make referrals to available services.
