Benchmark Human Services is hiring a Case Manager in the Vermont Springfield/White River Junction area to support individuals with intellectual and developmental disabilities and their families. The role focuses on planning, advocacy, monitoring, and coordinating services to promote independence and community inclusion.

The position requires VT residency, a valid driver’s license, and experience in human services or related fields.

Formación

• Bachelor’s degree in any field (or equivalent) plus 1 year of human services experience.
• Associate degree in human services or closely related field plus 2 years of human services experience.
• High school diploma or equivalent plus 3 years of human services experience.
• Must have knowledge of Vermont’s public service system for developmental disabilities and brain injury services.
• Must live in the state of VT.
• Must have a valid driver’s license and valid auto insurance.

Responsabilidades

• Complete core competency training required by the State.
• Perform evaluations and assessments to meet individuals’ needs.
• Assist individuals to understand goals and participate in person-centered planning.
• Schedule and facilitate person-centered planning meetings and assessments.
• Prepare needs assessments and annual review protocols with the individual and team.
• Help identify preferences, strengths, goals, and needs for planning.
• Facilitate service plan documentation and ensure information is available to the team.
• Support the individual and team in planning and rights modifications when needed.
• Monitor service plan implementation and health and welfare through visits and reviews.
