OneWell Health Care is seeking a compassionate Direct Support Professional (DSP) to provide hands-on, individualized support to individuals with IDD in home and community settings across Pennsylvania. You will assist with daily living activities, monitor health and safety, support transportation and community participation, and document care in approved systems under supervision of the Program Specialist.

This role requires English proficiency and a HS diploma; weekend and evening availability

Qualifikationen

• High school diploma or equivalent.
• Experience in caregiving or home health preferred.
• English proficiency required; bilingual is a plus.

Aufgaben

• Assist with daily living activities and personal care as outlined in care plans.
• Support mobility, meal preparation, transportation, and community participation.
• Monitor health, safety, and emotional well-being; document observations in approved systems.
• Maintain accurate records and participate in trainings and team meetings.
