Job summary

Qualifications

• Requires at least a bachelor’s degree in a human services-related field or a bachelor’s in a non-related field with at least one year of relevant work experience.
• Relevant experience means direct work with individuals with IDD, other disabilities, or mental illness.
• Proficiency in Microsoft Office and state-maintained databases; must complete training per program regulations.

Responsibilities

• Coordinate and monitor all services for each person on the caseload to ensure adequacy and health/welfare.
• Complete intake documentation for referred individuals and track Medicaid/waiver eligibility.
• Develop and implement each person’s Plan of Services and Supports (PSS) and update annually or as needed.
• Document all information in LTSS and perform required reporting, including grievances and approvals.
