Job summary

Mercy Home Health seeks a Clinical Nurse II to lead a Care Delivery Team, directing care for a group of patients with a focus on independent assessment, planning, implementation and evaluation. The CN II precepts new staff and demonstrates clinical leadership within Mercy’s nursing ladder.

The role requires Ohio RN licensure, completion of orientation, and ongoing CE (12 hours/year) including Fire Safety and CPR Re-Certification; ACLS is required.

Qualifications

• Graduation from a state-accredited school of nursing.
• Current licensure from the state of Ohio as an RN.
• Successful completion of general and unit-specific orientation and hospital probationary period.
• Maintains a minimum of 12 contact hours of continuing education annually including Fire Safety and CPR Re-Certification.
• ACLS certification is required.

Responsibilities

• Directs care for a team of patients as the leader of the Care Delivery Team.
• Precepts new employees as assigned.
• Coordinates nursing care for a specific group of patients across assessment, planning, implementation, and evaluation.
• Communicates with patients and families effectively and professionally.
• Supervises Licensed Practical Nurses and Patient Care Assistants.
• Reports to the Nurse Manager or Administrative Supervisor.
