Responsibilities

• Assess patient, family, and environment for safety risks and proactively address safety concerns.
• Provides ongoing, individualized education based on care plan and patient needs.
• Documents and communicates nursing care and patient status using SBAR and handoff processes.
• Performs nursing actions according to physician orders with critical thinking and accountability.
• Responds to emergency situations quickly and accurately to ensure patient safety.
