Job summary

Total Care Services, Inc. seeks a dedicated Supports Planner to identify, access, and coordinate home and community-based services for Maryland clients.

You will assist with waiver eligibility and plan development, advocating for clients while enabling independence and safe living at home. The role offers a hybrid work setup with home-based work and local visits; requires a 8:00am–5:00pm schedule, Monday–Friday, and a valid driver's license.

Qualifications

• Minimum age 21+ years.
• Must hold a Masters or Bachelors in Human services.
• Must be a licensed driver with own transportation and valid insurance.
• U.S. citizen or legally authorized to work in the United States.
• Proficient with MS Word and MS Excel; good communication skills.

Responsibilities

• Identify, access, and coordinate services to maintain clients’ health, safety and independence.
• Assist with waiver eligibility and maintenance.
• Get to know clients, advocate for them, and connect them with community services.
• Help clients understand options and develop a plan of service and goals.
• Arrange delivery of services and monitor ongoing support.
