Job summary

Zachary Piper Solutions is seeking a Supports Coordinator to join a healthcare and social assistance provider located in Chester, PA. The role manages a caseload of individuals with developmental disabilities and coordinates services to maintain health and safety.

Responsibilities include monitoring visits, needs assessments, and documentation of case information. A bachelor’s degree is preferred, and candidates should be able to travel locally and work across multiple counties.

Qualifications

• Experience in human services, case management, social services, behavioral health, or related fields is preferred but not required.
• Proficiency with Microsoft Office applications, including Excel.
• Excellent organization, time management, and follow-up skills with the ability to manage a high-volume caseload.

Responsibilities

• Manage an assigned caseload of approximately 40-50 individuals with intellectual and developmental disabilities.
• Conduct regular in-person monitoring visits with individuals, families, caregivers, and service providers.
• Coordinate and facilitate support services including transportation, employment assistance, job coaching, medical appointments, residential services, and community resources.
• Gather, document, and maintain detailed case information, utilizing monitoring tools and written reports.
• Perform ongoing follow-up with individuals, families, providers, and community resources to ensure services remain in place.
