Responsibilities

• Develop and maintain individualized service plans with individuals, families, providers, and teams.
• Manage a caseload of 40-50 individuals; travel within the territory for in-person visits.
• Coordinate, monitor, and evaluate authorized services to ensure appropriate care.
• Conduct routine monitoring visits and assess health, safety, and quality of life.
• Communicate proactively with providers and families; ensure timely follow-up.
• Maintain detailed case notes, referrals, and billing records per compliance standards.

Piper Companies is seeking a Supports Coordinator to join a mission-driven organization serving individuals with intellectual disabilities and autism throughout Spring City, PA. The Supports Coordinator will play a critical role in ensuring individuals receive the services, resources, and support needed to thrive in their everyday lives.

Responsibilities of the Supports Coordinator:

• Develop and maintain individualized service plans by collaborating with individuals, families, providers, and interdisciplinary teams to identify goals and support needs.
• Manage a caseload of approximately 40-50 individuals and travel throughout the assigned territory to conduct in-person meetings and service monitoring activities.
• Coordinate, monitor, and evaluate authorized services to ensure individuals receive appropriate care, resources, and community-based support.
• Conduct routine monitoring visits and assessments to evaluate health, safety, well-being, service utilization, and quality of life outcomes.
• Communicate proactively with providers, families, and support teams to address concerns, resolve issues, and ensure timely follow-up on identified needs.
• Maintain detailed documentation, case notes, referrals, service records, and billing requirements while following compliance standards.

Qualifications of the Supports Coordinator:

• Bachelor’s degree required, preferably in Psychology, Social Work, Criminal Justice, Human Services, or a related field.
• Strong passion for supporting individuals with intellectual disabilities, autism spectrum disorder, or other developmental disabilities.
• Excellent organizational, time management, written communication, and documentation skills.
• Proficiency with Microsoft Office applications, calendaring tools, and electronic documentation systems.
• Valid driver’s license, reliable personal vehicle, and willingness to travel within a 30-40 minute radius of the assigned office location.

Compensation for the Supports Coordinator includes:

• Salary range: $42,000-$46,000
