Qualifications

• Bachelor’s degree is required, preferably in related fields such as Psychology, Social Work, Criminal Justice, or Human Services.
• Strong passion for supporting individuals with intellectual disabilities, autism, or developmental disabilities.
• Excellent organizational and documentation skills, plus writing ability.

Responsibilities

• Develop and maintain individualized service plans with individuals, families, providers, and teams.
• Manage a caseload of 40-50 individuals; travel within the territory for in-person visits.
• Coordinate, monitor, and evaluate authorized services to ensure appropriate care.
• Conduct routine monitoring visits and assess health, safety, and quality of life.
• Communicate proactively with providers and families; ensure timely follow-up.
• Maintain detailed case notes, referrals, and billing records per compliance standards.
