Zachary Piper Solutions is seeking a Supports Coordinator to join healthcare and social assistance provider located in Chester, Montgomery County, and Delaware County, PA . The Supports Coordinator is responsible for managing a caseload of individuals with intellectual and developmental disabilities, conducting regular monitoring visits, assessing needs, and ensuring individuals receive the resources necessary to maintain their health, safety, and quality of life.

Responsibilities for the Supports Coordinator Include:

• Manage an assigned caseload of approximately 40-50 individuals with intellectual and developmental disabilities, ensuring their physical, medical, and support needs are being met.
• Conduct regular in-person monitoring visits with individuals, families, caregivers, and service providers to assess well-being, service effectiveness, and compliance with care plans.
• Coordinate and facilitate support services including transportation, employment assistance, job coaching, medical appointments, residential services, and community resources.
• Gather, document, and maintain detailed case information, utilizing monitoring tools and written reports to ensure accurate and timely recordkeeping.
• Perform ongoing follow-up with individuals, families, providers, and community resources to ensure appropriate services remain in place and identified needs are addressed.

Qualifications for the Supports Coordinator Include :

• Previous experience in human services, case management, social services, behavioral health, or related fields is preferred but not required.
• Proficiency with Microsoft Office applications, including Excel. (Preferred).
• Excellent organization, time management, and follow-up skills with the ability to manage a high-volume caseload.
• Ability to build professional relationships with individuals, families, providers, and community partners.
• Valid driver's license, reliable transportation, and willingness to travel locally.
• Bachelor's degree preferred in Psychology, Social Work, Criminal Justice, Human Services, or a related field.

Compensation for the Supports Coordinator Includes :

• Salary Range : $42,000 - $46,000**depending on experience**
• Mileage reimbursement provided
• Company-issued laptop, cellphone, and Remarkable tablet provided.
