Qualifications

• Bachelor's degree is required.
• Degree in Psychology, Social Work, Criminal Justice, Human Services, or related field preferred
• Valid driver's license, reliable personal vehicle, and automobile insurance required
• Ability to travel locally throughout assigned territory
• Strong written communication, organizational, and time management skills
• Passion for supporting individuals with disabilities and willingness to learn through extensive training

Responsibilities

• Manage a caseload of individuals with ID and ASD.
• Coordinate, monitor, and advocate for services that support health, safety, and quality of life.
• Conduct monitoring visits in homes, community settings, and day programs throughout Chester, Montgomery, and Delaware Counties.
• Connect individuals with services including transportation, residential supports, job coaching, and community programs.
• Maintain communication with individuals, families, and providers to ensure effective service delivery.
• Complete detailed documentation including case notes, monitoring reports, billing records, and compliance documentation

Education

Bachelor's degree

Degree in Psychology, Social Work, Criminal Justice, Human Services, or related field preferred
