Job summary

Qualifications

• Six months of experience as a County Caseworker 1 or equivalent
• Bachelor's degree with social welfare or related credits and one year of casework experience
• Equivalent combinations of education and experience in social services

Responsibilities

• Develop and update Individual Support Plans with individuals and their teams
• Coordinate needed services based on individual needs and goals
• Encourage individuals to explore community work opportunities when appropriate
• Complete initial Supports Coordinator training and ongoing learning activities
• Travel to meet with individuals and teams to review supports and progress
• Enter required data and maintain organized records

Education

Bachelor's degree with a social welfare major

Bachelor's degree with 12 credits in sociology, social welfare, psychology, gerontology, criminal justice or related social sciences and one year of professional social casework

Any equivalent combination of experience and training including 12 college credits in related social sciences
