Job summary

Alleghenies United Cerebral Palsy is hiring a Service Coordinator in Pennsylvania. The role partners with individuals and support teams to create, update, and manage personalized service plans, ensuring services meet needs and are delivered per the plan.

Hybrid/remote position with PA home visits; travel within PA may be required. Requires a Bachelor’s degree in a related field and 2+ years in case management with disabilities and autism. Tuition reimbursement and generous benefits are offered.

Qualifications

• Bachelor's degree in Social Work, Psychology, Education, Human Services, or related field.
• 2+ years of professional case management experience with people with physical disabilities.
• 2+ years of experience working with individuals with Autism.
• Knowledge of Medicaid Waiver programs, LTSS, HCBS preferred.

Responsibilities

• Develop individualized service plans (ISP) with clients and teams.
• Regularly update service plans to reflect changes and needs.
• Coordinate and monitor services with paid providers and community resources.
• Maintain open communication with support teams and ensure compliance with AUCP standards.
• Travel within the assigned region to meet participants and complete ISP visits.
