Job summary

Zachary Piper Solutions seeks a Supports Coordinator to join a healthcare and social services provider in Chester, PA. The role manages a caseload of individuals with developmental disabilities, conducts regular monitoring visits, assesses needs, and ensures access to resources to maintain health and quality of life.

Responsibilities include in-person visits with individuals, families, and providers; coordinating transportation, employment support, medical appointments, and community services;

Qualifications

• Bachelor's degree preferred in Psychology, Social Work, Criminal Justice, Human Services, or a related field.
• Experience in human services or case management preferred.
• Strong organization and follow-up skills.

Responsibilities

• Manage an assigned caseload of individuals with intellectual and developmental disabilities, ensuring their physical, medical, and support needs are met.
• Conduct regular in-person monitoring visits with individuals, families, caregivers, and service providers to assess well-being and care plan effectiveness.
• Coordinate and facilitate support services including transportation, employment assistance, job coaching, medical appointments, residential services, and community resources.
• Gather, document, and maintain detailed case information, utilizing monitoring tools and written reports for accurate records.
• Perform ongoing follow-up with individuals, families, providers, and community resources to ensure services remain in place.
