Job summary

MultiCare Health System is seeking a Nurse Navigator RN to provide comprehensive services across the care continuum. The role blends nursing, case management and patient liaison duties, empowering you to self-direct and prioritize for better patient outcomes.

Qualifications

• Bachelor's degree in Nursing or RN with 15+ years of experience may be considered in lieu of education (employees hired before 11/30/2025 exempt).
• Advanced degree in Nursing (MSN, PhD, DNP, ARNP) preferred.
• Current RN license in Washington State or MLS endorsement.
• OCN/CCMC/ACMA or similar certification desirable.
• 3–5 years of recent clinical experience in area of specialty required.
• Current BLS certification.

Responsibilities

• Organize patient care by prioritizing, delegating, delivering, and evaluating on an ongoing basis.
• Engage critical thinking to make decisions regarding patient care using standards, resources, and data.
• Utilize nursing knowledge to deliver care using the nursing process per policies and protocols.
• Collaborate with the nursing team to improve quality of care, patient experience, and reduce costs.
• Foster a learning environment for patients, families, nursing staff and healthcare team.
• Advocate for ethical, holistic care respecting diverse backgrounds.
• Track key quality metrics using national benchmarks where applicable.
• Lead care team initiatives to improve processes and outcomes aligned with the quadruple aim.
• Address non-clinical barriers by connecting patients with community services.
