Job summary

MultiCare Health System is seeking an Ambulatory RN Care Manager to coordinate across the health system, ensuring safe transitions and quality patient care. You will use nursing process to deliver patient-centered care in ambulatory settings and work with diverse teams to improve population health.

Qualifications

• Bachelor's degree in Nursing or obtained within five years from the initial date of hire; RN with 15+ years of service or internal candidates hired before 1/1/2021 may be considered instead of an education agreement
• Current RN license in Washington State or Multistate license endorsement (MLS)
• Master’s degree in nursing preferred
• CCMC/ACMA/AAACN ambulatory care nursing certification or similar required or obtained within 3 years of hire (waived for some employees)
• Three years of recent clinical hospital/ambulatory experience
• Knowledge of the care continuum (inpatient, ambulatory, post-acute) preferred

Responsibilities

• Ensure patients and families experience seamless and safe transitions of care
• Advocate for patients and families with sensitivity to cultural and ethnic practices
• Provide patient-centered education on disease processes and wellness opportunities
• Assist patients and families toward self-management of care and goal setting for chronic disease
• Coordinate community resources via nurse visits, telephone management or other contacts
• Collaborate with leadership and care team to optimize resource utilization across the continuum
• Serve as resource for staff, physicians and providers to impact CMS core measures and NP goals
