Job summary

CareSource is seeking an Integrated Care Clinical Manager to oversee community-based care for dually eligible enrollees with complex needs. The role focuses on coordinating medical, behavioral, and social services to improve outcomes and quality of life.

Regularly travels to member homes and community settings to ensure seamless care transitions. The position requires an RN license, AS nursing degree, and experience in care coordination.

Qualifications

• Associates of Science (A.S) degree in nursing from an accredited program.
• Active RN license with ability to independently serve people with complex needs.
• Experience in care coordination or case management preferred.

Responsibilities

• Engage with enrollees in their homes and community settings.
• Coordinate care across providers and community resources.
• Lead interdisciplinary care team to create holistic care plans.
• Ensure enrollee utilization of long-term services and supports.
• Educate members about benefits, services, and chronic condition management.
• Travel to member, provider, and community visits as required.
• Maintain compliance with NCQA and Care Management standards.
