Job summary

MultiCare is seeking an Ambulatory RN Care Manager to coordinate cross-continuum care and transitions for ambulatory patients. You will apply nursing process, collaborate with teams, and focus on safe, patient-centered care to improve population health.

The role emphasizes organization, clinical competency, and the ability to guide patients and families toward better health outcomes within the MultiCare system.

Qualifications

• Bachelor's degree in Nursing required.
• Current RN license in WA or MLS.
• 3 years of recent clinical hospital/ambulatory experience.
• Certification in CCMC/ACMA/AAACN or similar preferred within 3 years.

Responsibilities

• Coordinate cross-continuum care and transitions of care.
• Advocate for patients and families with cultural sensitivity.
• Educate patients on disease processes and wellness opportunities.
• Assist with self-management plans and community resource coordination.
• Collaborate with leadership to optimize resource utilization.
• Serve as resource to staff and providers to improve CMS core measures.
