Qualifications

• Active Florida RN or LPN license.
• Strong communication and interpersonal skills.
• Detail-oriented and organized.
• Valid driver’s license and reliable transportation.

Responsibilities

• Build relationships with case managers and hospital staff.
• Educate referral sources on home health services and programs.
• Coordinate care from hospital to home and support admissions goals.
• Educate patients and families about coverage and services.
