Job summary

Enlyte is seeking a Field Case Manager in the New Haven, CT area for a full-time, hybrid role requiring regular local travel to in-person patient appointments. You will work with treating physicians, employers, and injured workers to craft and implement personalized treatment plans that promote safe, cost-effective care and timely return to work.

Responsibilities include applying case management standards, evaluating injuries, collaborating with providers, and adjusting goals based on progress.

Qualifications

• RN licensure in good standing is required.
• 2+ years clinical practice preferred; workers’ compensation experience preferred.
• Strong written and verbal communication; proficient in basic computer skills.
• URAC-recognized CCM/CDMS/CRC/CRRN or related certifications preferred.

Responsibilities

• Develop and implement treatment plans with physicians/providers to facilitate early return to work.
• Coordinate care with employers, customers, and injured workers to ensure cost-effective healthcare.
• Attend injured workers’ appointments and prepare status updates for customers as needed.
• Manage workload independently, prioritizing cases and adapting goals as recovery progresses.
