Job summary

Enlyte in New Haven, CT is seeking a Field Case Manager to coordinate treatment plans with physicians, employers, and patients. This is a full-time, hybrid role with regular local travel for in-person appointments.

Requires an RN license with 2+ years clinical experience; URAC certification preferred. Benefits include medical, dental, vision, and 24 days of vacation, plus travel reimbursement and equipment.

Qualifications

• Education: Associates or Bachelor’s Degree in Nursing or related field.
• Experience: 2+ years clinical practice preferred; workers’ compensation experience preferred.
• Skills: Advocate recommendations with physicians, employers, and customers; work independently; basic MS Office.
• Certifications, Licenses: Active RN license in good standing; URAC‑recognized case management cert preferred.
• Travel: Reliable transportation to attend in-person appointments.
• Internet: Reliable internet access.

Responsibilities

• Create and implement treatment plans that return injured/disabled workers to work.
• Collaborate with treating physicians/providers and employers to tailor plans.
• Modify job duties with employers to facilitate early return to work when possible.
• Assess recovery progress and adjust case goals accordingly.
• Independently manage workload and document status updates for clients.
